About the Client:
We are a caring family dedicated to supporting our 50-year-old mother who is a breast cancer survivor facing long-term effects of chemotherapy. Our primary objective is to provide her with comprehensive care and support to address the physical and emotional challenges of survivorship and promote her overall well-being and quality of life.

Project Overview:
We are seeking an experienced caregiver with expertise in cancer survivorship care to provide personalized support for our mother. This project entails ongoing management of treatment-related side effects, survivorship care planning, and support for addressing physical and emotional needs associated with her cancer journey.

Responsibilities:

Monitor and manage treatment-related side effects, such as fatigue, neuropathy, and cognitive changes, through symptom assessment, medication management, and supportive interventions to enhance our mother’s comfort and quality of life.
Collaborate with healthcare providers to develop and implement survivorship care plans tailored to our mother’s unique needs and preferences, addressing ongoing health monitoring, surveillance, and supportive care services to optimize her long-term health outcomes and survivorship experience.
Provide emotional support and encouragement to our mother, addressing her concerns, fears, and emotional distress associated with cancer survivorship, and facilitating access to counseling, support groups, and other psychosocial resources as needed.
Assist with lifestyle modifications and supportive interventions to promote our mother’s overall health and well-being, including nutritional counseling, exercise recommendations, stress management techniques, and strategies for enhancing sleep quality and fatigue management.
Facilitate communication and coordination of care among healthcare providers, specialists, and support services involved in our mother’s cancer survivorship journey, ensuring seamless transitions and continuity of care across the healthcare continuum.
Requirements:

Experience in oncology nursing or cancer survivorship care, with a strong understanding of the physical and psychosocial aspects of cancer survivorship and evidence-based interventions to address treatment-related side effects and promote long-term wellness and quality of life.
Compassion, empathy, and sensitivity to the emotional and psychological needs of cancer survivors and their families, fostering a supportive and nurturing environment conducive to healing and recovery.
Excellent communication and interpersonal skills, with the ability to build trust, rapport, and collaborative relationships with cancer survivors, family members, healthcare providers, and community resources involved in survivorship care.
Knowledge of survivorship care planning principles and practices, including survivorship care plan development, implementation, and evaluation, to ensure comprehensive and individualized care that meets the unique needs and preferences of our mother.
Commitment to ongoing professional development and education in oncology nursing and cancer survivorship care, staying abreast of emerging research, best practices, and advancements in cancer care and survivorship support services.
