Project Overview:
We are seeking an experienced nurse to join our caregiving team on a contractual basis, with a focus on providing specialized care for our 65-year-old family member living with terminal liver cirrhosis. This role involves providing palliative care, managing distressing symptoms such as ascites, hepatic encephalopathy, and pruritus, and offering support and guidance to family caregivers to provide comfort and assistance to our loved one during this difficult time.

Responsibilities:

Palliative Care: Provide compassionate palliative care to our family member, focusing on alleviating physical discomfort, promoting emotional well-being, and enhancing overall quality of life. Offer comfort measures, pain management interventions, and supportive therapies to address their symptom burden and improve their comfort and dignity as they approach end-of-life care.
Symptom Management: Assess and manage distressing symptoms associated with terminal liver cirrhosis, including ascites, hepatic encephalopathy, and pruritus. Utilize evidence-based interventions, pharmacological therapies, and non-pharmacological approaches to alleviate symptoms, relieve suffering, and improve our loved one’s comfort and functional status, ensuring their needs are addressed comprehensively and compassionately.
Family Caregiver Support: Offer guidance, education, and emotional support to family caregivers as they provide care and assistance to our loved one. Provide training on symptom management techniques, medication administration, and comfort measures to empower caregivers to meet our family member’s needs effectively and confidently, while also attending to their own well-being and coping with the emotional challenges of caregiving.
Communication and Collaboration: Foster open communication and collaboration with other healthcare professionals involved in our loved one’s care, including physicians, social workers, and hospice providers. Participate in interdisciplinary care team meetings, share information, discuss treatment goals, and coordinate care plans to ensure a holistic and coordinated approach to our loved one’s care, promoting continuity and effectiveness of care delivery.
Emotional Support: Offer compassionate emotional support and companionship to our loved one and their family as they navigate the challenges of terminal illness and end-of-life care. Provide a listening ear, validation of feelings, and guidance on coping strategies to help them process their emotions, find meaning and comfort in their relationships, and foster a sense of peace and acceptance during this difficult time.
Advance Care Planning: Facilitate discussions on advance care planning with our loved one and their family, helping them articulate their goals, values, and preferences for end-of-life care. Assist in completing advance directives, documenting healthcare wishes, and ensuring their care preferences are honored and respected throughout the course of their illness, promoting autonomy and dignity in decision-making.
Requirements:

Certification as a registered nurse with experience in palliative care, hospice care, or end-of-life care.
Proficiency in managing distressing symptoms associated with terminal liver cirrhosis, including ascites, hepatic encephalopathy, and pruritus, through evidence-based interventions and symptom management techniques.
Strong communication and interpersonal skills, with the ability to provide compassionate care and support to individuals and families facing the challenges of terminal illness and end-of-life care.
