M.D., D.O., or foreign equivalent. Completed residency in Internal Medicine required. Illinois License to practice medicine is required. Board eligibility in Internal Medicine required. Federal Drug Enforcement Agency License and State of Illinois License for prescribing of controlled substances required. Must complete physical examination and drug and alcohol screening.

Special Requirements Valid drivers license and travel to various households in the Houston, Texas area. Police background check and negative covid test upon hire. Must have letter of reference.
