We are a community organization committed to supporting individuals with intellectual disabilities in living independently and thriving in their communities. Currently, we are assisting a 30-year-old woman with an intellectual disability who is living independently but requires support with meal planning, budgeting, and accessing community resources. Our goal is to empower her to maintain her independence while ensuring she has the necessary support to meet her daily living needs. We are seeking nursing assistance to provide tailored support and guidance for her specific requirements.

Project Overview:
We are seeking an experienced nurse to join our community support team on a contractual basis, with a focus on providing specialized care for a 30-year-old woman with an intellectual disability living independently. This role involves offering support with meal planning, budgeting, and accessing community resources to promote her independence and overall well-being.

Responsibilities:

Assessment and Planning: Conduct a comprehensive assessment of the individual’s needs and capabilities related to meal planning, budgeting, and accessing community resources. Develop a personalized care plan tailored to her specific requirements, preferences, and goals for independent living.
Meal Planning Support: Offer support and guidance to assist the individual with meal planning, including menu selection, grocery shopping, and preparation of nutritious meals. Provide education on healthy eating habits, portion control, and food safety practices to promote her physical health and well-being.
Budgeting Assistance: Assist the individual with budgeting her finances effectively, including income management, bill payment, and tracking of expenses. Offer strategies for financial planning, saving, and prioritizing spending to ensure she can meet her basic needs and achieve her financial goals.
Community Resource Access: Help the individual access community resources and support services that align with her needs and interests. Provide information and referrals to local organizations offering social activities, recreational programs, vocational training, and other opportunities for social engagement and skill development.
Health Promotion: Promote the individual’s overall health and wellness by offering guidance on preventive healthcare practices, self-care activities, and accessing medical services as needed. Advocate for her rights to healthcare access and ensure she receives appropriate medical care and support.
Collaboration: Collaborate closely with other members of the community support team, including social workers, case managers, and community advocates, to ensure coordinated and holistic care for the individual. Communicate regularly with team members to provide updates on her progress, address concerns, and adjust care plans as needed.
Requirements:

Certification as a registered nurse with experience in community health nursing, disability support, or independent living assistance.
Proficiency in assessing and addressing the unique needs of individuals with intellectual disabilities, including knowledge of strategies for promoting independent living skills and community integration.
Strong communication and interpersonal skills, with the ability to build rapport and trust with individuals with disabilities and their support networks.
Compassion, empathy, and a genuine commitment to empowering individuals with intellectual disabilities to live fulfilling and independent lives.
