Client Overview:
We are a compassionate community organization dedicated to providing support for individuals facing homelessness and mental health challenges. We are currently assisting a 40-year-old adult living with schizophrenia who is experiencing homelessness. Our primary goal is to offer outreach services, medication management, and support for accessing housing and community resources to help them stabilize their situation and improve their quality of life. We are seeking skilled nursing assistance to join our outreach team and provide the specialized care and support needed for this individual.

Project Overview:
We are seeking an experienced nurse to join our outreach services team on a contractual basis, with a focus on providing specialized care for a 40-year-old adult living with schizophrenia who is experiencing homelessness. This role involves medication management, coordination of care with community resources, and offering support to help them access housing and other essential services.

Responsibilities:

Outreach Services: Provide outreach services to locate and engage with the individual living with schizophrenia who is experiencing homelessness. Establish rapport, build trust, and offer support to encourage them to access healthcare services and participate in treatment programs.
Medication Management: Assist in managing medications prescribed for schizophrenia, ensuring adherence to treatment plans, monitoring for potential side effects, and coordinating medication refills as needed. Educate the individual on the importance of medication compliance and provide support to address barriers to adherence.
Coordination of Care: Collaborate with community resources, including homeless shelters, mental health clinics, and social service agencies, to coordinate care and facilitate access to housing, healthcare, financial assistance, and other essential services. Advocate for the individual’s needs and rights within the community.
Support for Accessing Housing: Offer support and guidance to help the individual access housing options, including transitional housing programs, supportive housing arrangements, and rental assistance programs. Assist with completing housing applications, navigating housing resources, and advocating for stable housing solutions.
Community Resources: Provide information and referrals to community resources and support services, including mental health counseling, substance abuse treatment, vocational rehabilitation, and peer support groups. Help the individual connect with resources that promote stability, recovery, and overall well-being.
Collaboration: Collaborate closely with other members of the outreach services team, including case managers, social workers, and community advocates, to ensure coordinated and comprehensive support for the individual. Communicate regularly with team members to provide updates on the individual’s progress, address concerns, and adjust care plans as needed.
Requirements:

Certification as a registered nurse with experience in psychiatric nursing, community health nursing, or outreach services.
Proficiency in managing medications for individuals with schizophrenia, including knowledge of antipsychotic medications, side effects, and medication adherence strategies.
Strong communication and interpersonal skills, with the ability to engage effectively with individuals experiencing homelessness and mental health challenges.
Compassion, empathy, and a genuine commitment to improving the lives of individuals facing homelessness and mental illness.
