Project Overview:
We are seeking an experienced nursing team with expertise in palliative care and pain management to deliver compassionate and holistic care. This project focuses on implementing pain management strategies, providing emotional support, and ensuring the client’s comfort and dignity.

Responsibilities:

– **Pain Management:**
– Administer pain medications, including opioids and adjuvant therapies, as prescribed, ensuring timely and effective pain relief.

– Facilitate communication between the client, her family, and healthcare providers to ensure that her wishes and preferences are respected and incorporated into her care plan.

– **Palliative Care Coordination:**
– Develop and implement a comprehensive palliative care plan that includes symptom management, emotional support, and end-of-life care.
– Coordinate with the palliative care team, including physicians, social workers, and chaplains, to provide a multidisciplinary approach to care.
– Ensure seamless transitions of care and continuity between home, hospital, and hospice settings as needed.
