About the Client:

Our primary goal is to help him regain motor function and communication abilities through comprehensive rehabilitation services provided by a skilled nursing team.

Project Overview:
We are seeking an experienced nursing team to provide intensive care and rehabilitation for our loved one. This project involves delivering physical therapy, speech-language therapy, and assistive technology interventions to enhance motor skills, speech, and overall quality of life, enabling him to achieve the highest possible level of independence.

Responsibilities:

– **Physical Therapy:**
– Conduct thorough assessments of the patient’s current physical abilities and limitations.
– Develop and implement a personalized physical therapy plan focused on improving strength, coordination, balance, and mobility.
– Utilize therapeutic exercises, gait training, and functional activities to enhance motor function and prevent complications such as contractures and muscle atrophy.
– Monitor progress and adjust the therapy plan as needed to achieve optimal outcomes.

– **Speech-Language Therapy:**
– Evaluate the patient’s speech, language, cognitive-communication, and swallowing abilities.
– Create an individualized therapy plan to address aphasia, including exercises and strategies to improve language comprehension, expression, and speech production.
– Provide training in alternative communication methods and use of speech-generating devices if necessary.
– Collaborate with family members to support communication at home.

– **Assistive Technology:**
– Assess the patient’s needs for assistive devices and technology to aid in daily living activities and communication.
– Recommend and provide training on the use of devices such as communication boards, speech-generating devices, mobility aids, and adaptive equipment for daily tasks.
– Ensure the patient and family are comfortable and proficient in using the assistive technology to enhance independence and quality of life.

– **Family Education and Support:**
– Educate family members on the patient’s condition, therapy plans, and ways to support recovery at home.

Requirements:
