Project Overview:
We are seeking compassionate and experienced nurses to provide comprehensive care for our family member who is living with advanced chronic heart failure. This project entails a 6-month contract with the possibility of extension based on the evolving needs of her health and care requirements.


Provide attentive and specialized care tailored to the needs of a patient with advanced heart failure.
Monitor for signs of fluid overload and other complications, ensuring timely intervention.
Administer medications and follow prescribed treatments as directed by healthcare professionals.
Implement dietary management plans to support heart health and overall well-being.
Conduct daily weight checks to monitor for sudden changes that could indicate fluid retention.
Collaborate closely with family members and other caregivers to maintain a cohesive and supportive care plan.

Certification as a registered nurse with a minimum of 3 years of relevant experience in cardiac or heart failure care.
Proficiency in managing heart failure symptoms, medication administration, and dietary planning.
Strong understanding of heart failure-related health conditions and management strategies.
Ability to work independently and as part of a team in a home environment, demonstrating flexibility and adaptability to the unique needs of the patient.
Excellent communication skills and a compassionate demeanor, fostering a supportive and collaborative relationship with the family and other caregivers.
Patient Case:

Chronic Heart Failure

Patient: 78-year-old female

Condition: The patient is living with advanced chronic heart failure. She requires comprehensive and ongoing care to manage symptoms, prevent complications, and support her daily living.

Care Plan:

Monitoring for Fluid Overload: Regularly monitor for signs of fluid overload, such as swelling, shortness of breath, and weight gain. This includes daily assessments and adjustments to care plans as needed.
Medication Administration: Ensure timely administration of prescribed medications, monitoring for side effects and effectiveness. Adjust medications as directed by healthcare professionals.
Dietary Management: Implement and monitor dietary plans tailored to the patient’s needs, focusing on low-sodium and heart-healthy foods to manage fluid retention and support overall health.
Daily Weight Checks: Conduct daily weight checks to monitor for sudden changes that could indicate fluid retention or other complications, and report findings to healthcare providers promptly.
