Project Overview:
We are seeking an experienced nurse to join our caregiving team on a contractual basis, with a focus on providing specialized care for our elderly couple living with advanced dementia and sundowning. Provide training on dementia care strategies, communication techniques, and coping skills to empower family caregivers in meeting the unique needs of their spouses, while also attending to their own well-being and managing caregiver stress and burnout.
Collaboration: Collaborate closely with other healthcare professionals involved in our elderly couple’s care, including physicians, geriatricians, psychologists, and occupational therapists, to ensure coordinated and holistic support. Communicate regularly with the interdisciplinary team to share information, discuss care goals, and address any emerging needs or concerns, promoting continuity and effectiveness of care delivery.
Progress Monitoring: Regularly assess our elderly couple’s cognitive function, behavioral symptoms, and response to interventions to evaluate the effectiveness of care strategies and identify areas for adjustment or modification. Use standardized assessment tools, behavioral observations, and family feedback to monitor changes in their condition, optimize their care plans, and enhance their overall well-being and quality of life.
