Responsibilities:

Provide attentive and specialized care tailored to the needs of a patient with chronic heart failure.
Implement and support lifestyle modifications to promote heart health and overall well-being.
Administer medications and follow prescribed treatments as directed by healthcare professionals.
Monitor the patient’s overall health conditions and vital signs to ensure stability and safety.
Supervise the patient to prevent falls and other health-related incidents, ensuring a secure living environment.
Provide emotional support to help the patient cope with anxiety related to her condition and mortality concerns.
Collaborate closely with family members and other caregivers to maintain a supportive and cohesive care plan.
Requirements:

Certification as a registered nurse with a minimum of 3 years of relevant experience in chronic heart failure or geriatric care.
Proficiency in managing chronic heart failure and associated anxiety.
Strong understanding of heart failure-related health conditions and lifestyle modifications.
Ability to work independently and as part of a team in a home environment, demonstrating flexibility and adaptability to the unique needs of the patient.
Excellent communication skills and a compassionate demeanor, fostering a supportive and collaborative relationship with the family and other caregivers.

Chronic Heart Failure

Patient: 85-year-old woman

Condition: The patient is living with chronic heart failure, experiencing significant anxiety related to shortness of breath and concerns about mortality. She requires comprehensive and ongoing care to manage her symptoms, medication, and overall well-being.

Care Plan:

Medication Management: Ensure timely administration of prescribed medications, monitoring for side effects and effectiveness.
